What did you put for the ban? They might be coming from an ip that changes each time they logon (like with dialup) so you'll need to use a mask.

Example:

Say I posted some nasty comment on your forum and my ip was 192.168.1.1, so you banned that address. Next time I posted something but I was coming from 192.168.1.4.

Then you'd have to ban 192.168.1.* in order to get rid of me.
