I experience disconnect on a regular basis with my manager. For example, she will delegate a simple task such as delivering a file to someone. She won't imply or state that this task is urgent. Just that it needs to happen. Usually, delivering files is not an urgent matter. Therefore,I would put something like that on my back burner and get to it after lunch.

My manager would then ask me about the task and be surpised if I haven't taken care of it. I wasn't involved in any of the conversations, therefore, I would not realize that the file was needed for a meeting that the person was going to be preparing for later that day. So, my disconnect is with her delegating tasks for me to complete but her not giving all the information (i.e, complete instructions complete with a timeframe in what the task needs to stay in).

I consider this disconnect a display of poor management and communication skills on her part. But of course she would state that it should be my job to inquire about such things if she doesn't supply them. Obviously I feel differently.
