Well, Fuuma did have a dream of Kamui killing Kotori, didn't he? Or was that just movie stuff? *doesn't recall; it's been a while since she looked at the early manga* But there's no reason for Subaru to know about that, so maybe it's just an implication that Kamui would take the blame to spare Fuuma the knowledge? Even if Kamui and Fuuma did switch places, Kotori's already dead, and the identity of the one who killed her isn't going to change.

As for Flower, it's a bathhouse. From my understanding, it's a place where men go to be bathed by scantily-clad or outright naked women. The furthest it gets is oral sex on the man. So, Karen's not really a prostitute, at least in the traditional sense. Bathhouses are, as far as I know, as close to prostitution as is legal in Japan.
