Sure the blue handed guys were sloppy, but who's going to tell on them? Everyone (aside from Mal's people) who possibly saw them is now dead. Also they may have, most likely have, the power to wipe/alter any security cameras in that area of the hospital.

Perhaps the blue hands were framing River. Imagine if they spread the rumors that the sloppy sonic deaths were actually caused by the very powerful "fugitive" River and her brother Simon. If they can prove she's a real danger there may be more incentive for people to capture her.

My guess is the blue hands could afford to be sloppy because they know the blame isn't going to fall on them.
