I guess a lot depends on how much of a stickler for the law you want to be. Technically, as a live-out nanny, you are entitled by Federal Law for time and half for ALL hours worked past 40 hours per week, regardless of when (at night) or where (on vacation or at home) they are worked. But, since most families would freak over the actual amount that this entails, the majority of nannies come up with a flat fee that covers it. Because I do not like to travel with families (I am married and would rather have my own life), I charge 24/7 and overtime is a factor for me. It usually means I only go if they really need me! You have to decide what you would truly be comfortable with, and my guess is based on what you are saying, you would rather not go. No amount of money is worth a week of been bored, lonely, and undermined.
