Personally I would so the splint first, especially if you have facial pain and tinitus. This will "free up" the musculature and allow for for comfortable treatment.

What I meant by leave composite shelves is on the splinted retainer i will leave an extra bit of composite material so thet your lower front teeth has something to hit on. This way the bite will not close back down. This is often done in deep bite TMJ cases.
