Have you had her checked out by a vet? Could be a physical problem (the urination etc).

Regarding the housetraining, you need to think back very carefully exactly how the original puppy training went - somewhere back there lies the answer to your current problems. Was training always consistent? Was reward given everytime she did it right? Did you really take time and effort to show her exactly where she should go, and reward her when she did it right? Did anyone EVER train her negatively when she did it in the wrong place?

Regarding the chewing etc (and the potty problems actually) the only solution at this time is to limit her access to the house for some time. This does NOT mean she should be shut outside. It does mean that for a while, until things go better and she can be trusted more, you limit WHERE she goes. This means, for example, limiting her to the kitchen or another convenient place UNLESS she is physically in your presence. In other words, set her up to succeed - if she cannot get at the shoes etc, she won't chew them. Then train her not to, by for example distracting her when she's with you and heads to those shoes, with a toy.

Crate-training might also be beneficial in this case. In other words, limit her access unless she is with you.

The final suggestion from my side would be to contact a reputable canine behaviour psychologist and get some professional help aimed specifically at your situation.
