The DVD commentary mentions the electrical muscle exercising machine beside the bed in that scene, and notes that it was a clue to Locke's unrevealed disability which most people didn't notice, and also an important reason why Locke's muscles had not decayed (as scripted by David Fury).

They also mentioned other clues, and said that they didn't "cheat" except by reducing the height of the wheelchair handles in that scene in the adventure tour company's office in Sydney so that it wouldn't be obvious that he was in a wheelchair until he moved at the end of the scene.
