This is called Lateral Line and Head Erosion (hole in head disease)... The causes are unclear. Make sure the water quality is optimal and reduce stress. Stopping carbon filtration may help as it can remove nutrients from the water... it seems to be related to a vitamin C deficiency. Thus, feed a vitamin enriched food (with lots of C).

Some sources recommend metronidazole to eliminate Hexamita from the lesions... but it is not necessarily a cure-all. I wish you the best of luck..
