For online mailing to Universal; when copying and pasting the DVD form--start with Dear Colleen..to the end, and sign your name.

When sending by snail mail, print it out, cut any ads or stuff off, sign your name, paper clip and fold it up, and stick it inside an envelope and send it of. I did both of the above methods.
