Definitely stay on the island and enjoy its beauty and peacefulness, there's really no need to go anywhere else. My husband and I have been going to Sanibel for 13 years (next month will be our 14th..YIPPEE!!), have eaten in quite a few restaurants, and have yet to be disappointed. Twilight Cafe is excellent. There's no need to worry about getting dressed, everywhere is casual. Another place on the island you may want to consider visiting is the historical village, it's located by the library. We finally went there last year and really enjoyed it, the buildings are fairly close so you can visit all of them, the docents are knowledgeable and friendly. Enjoy your stay, I'm sure you'll have a wonderful time.
