If the D.O.C. suspects you of abusing your sick time they can request documentation. The request must be made in writing utilizing the department's attatchment "D". Once you receive the attatchment "D" you have seven days to provide a note. That's when you need to contact your doctor and ask him to write you a note. Tell him exactly how to write the note and don't settle for anything else.
