Hey, Michelle, great example! Whenever trying to explain a piece of theory, examples make it much more comprehensible!
Mabel, I think that the ideas of discourse organization, pragmatics and grammatical choice are strongly linked; what I tried to say is that, although they "depend" on one another, they represent different concepts, different elements at the discourse level (of course I may be wrong).
As for exophoric reference, I could say that it's the external world the author expects you to share with him. This information is not in the text, it may be on your knowledge of the world - at least the author expects so - and, depending on the level of world knowledge you share with the author, he may or may not put his message across. An interesting example: Rubem Fonseca, one of my favourite authors, mentions specific places around the world and uses other languages in his books which he does not translate - he expects his readers to share all this knowledge with him, and sometimes we have to either ignore some details or to search for them in order to keep going...
