It looks like the bigger boys had a surgery in the lower respiratory organs or even the heart with the purpose of enhancing sport performance.

It is difficult to know what was done, but possibly one or more of this:

Enhancing of the volumes of the thrakea or broncs to let more air pass.

Cutting of nerves to reduce asthmatic reaction during hard performance. These reaction is not due to asthma as such, but is a reaction upon exaggerated strain.

Perhaps even bypass surgery in the hart to enhance the blood stream.

In either case, this is a blatant example of the practice of doing whatever they like with the kids to create top winning athletes.
