From this I deduced that these boys had surgery in their chest, and speculated what that could be.

But then you wrote something that indicate that they had open abdominal surgery, and wondered if that was because they had outie navels that necessitated open surgery in their abdomen to take out the appendix.

Did these boys have scars both on abdomen and chest?

If the open surgery was in the abdomen, I think they performed a more extensive reworking of the lower abdominal organs.

Did you see these older boys laying in intensive care unit?

Did they have more tubes, electrodes and the like going inside them? Were they connected to more machines?

If so, where in their bodies did these things enter? That may give a clue to what was done with them.
