I totally agree that it is the mother who knows her son best and has the right to decide with what means to secure the obedience and co-operativeness of her son during the most intimate part of the examination. Even when he shows obedience, it is desirable to be in close proximity for traces of his behavior and to ensure with her's parental authority co-operation and empathy to the whole procedure. It is also a great opportunity for to refresh her impressions of her son's development and to alert timely about something unusual or worrying. I do not think bare bottom spanking of a young man who is already well in puberty is a good idea, even less in public with audience.
