Japan has a lot of small ambulances for one good reason -- Japanese roads are VERY narrow.

I rode in a Japanese ambulance once. An Australian ambulance would never have been able to negotiate its way into the streets around the Iwadono Ward where I lived.

To give you an idea of just how narrow the roads are -- on Australian roads, when you make a left turn, you just casually turn the wheel about 180 degrees or so and start turning back as your negotiate your way through the corner. On Japanese roads -- especially small residential ones, the corner is so tight that you have to literally turn the wheel all the way to the left and LOCK it -- and even then, you only JUST make the turn! Needless to say that driving in Japan freaked me out. ;p

One interesting thing that Japanese ambulances do though is that as well as having a siren wailing, they have a loudspeaker where they will literally announce that there is ambulance coming through and ask all drivers to make way whenever they approach an intersection. At first I thought it was strange, but actually it's a pretty good idea considering how many times we see some stupid drivers ignoring the sounds and lights of your conventional siren. Sure, it's not foolproof -- you could also ignore the announcement too, but I think it's good that there's just an extra element in letting other drivers know that there's an ambulance coming through.
