A lot of players go through this sort of problem big or small, to rebuild self-efficacy (confidence in performing a skill) in the tackling there are a few avenues open to you:
1. Wear a scrum cap and shoulder pads
2. Start from scratch tackle drills on a one to one basis
and gradually build up the difficulty e.g. start the tackler in a kneeling position and build up from there, with very little resistance from the tackled player.
3. Use tackle pads to get the body positions right
4. Use reinforcement and positive encouragement as much as possible, make it clear that they were once a brave tackler and they have only forgotten how to do it, fear is a big block to many things that we enjoy doing.

It will take time to go through the drills, but whilst the team are doing contact drills your player can work with another player on these drills and when ready integrate back in.
