feel for epigastric tenderness with palpation of the abdomen, boardlike abdomen and rebound tenderness (significant for perforation). Look for hematemesis or melena, occult blood ( ulcers may bleed). Listen for hypoactive bowel sounds if pyloric channel is obstructed, or patients history, description of pain, N?V, dark stools, PMH of ulcers, taking alot of steroids or NSAIDS, smoking, alcohol, etc. listening to your paitent history can sometimes point you to the diagnosis faster than the actual exam. hope this helps, good luck!!
