I did win my case on the first round... i did a letter about everything i have done in the last 10 months.

But the below says everything i would have done in round 2... keep copies... if there is something written wrong from them...correct it copy it and then give it back to them... anything and everything you can get your hands on about FM, CFIDS, MPS, IBS, RLS.... send them proof that these illnesses exist and something explaining things in detail... i sent about 10 atricles on fm, then depression, anxienty, Parkinson's, ibs, mps, rls...anything and everything i could get my hands on.

Okay… here goes…I just got off the phone with SSDI and SSI…I am all signed up for both… and I have done everything to get the money in my account asap… they just got my file so will have to wait to see how many back months I get but here is what I was told and though I should share… this is important…

SSDI said as soon as my file gets back they will do the money thing and I don’t have to come in to sign or do anything…

SSI said I needed to have another interview with them and I have to sign and send the paperwork they send me from interview… just did that interview…whee.

Now ssi told me how all of this works… on you back pay you only get the last 12 months of back pay… so for me – if I am found disabled from Nov. 2001, there is a 5month waiting period before benefits start… so may 2002 is when they would start but since I only get the last 12 months some of the time I did not work is lost… no matter what you can only get the last 12 months unless there was a denial in your pervious case… hope you can understand that… he explained it all to me… it is awesome to find someone with the information… still a waiting game for my file but it is all good… knowing there will be money coming in is so so so very helpful. Smiles…

Below is some information about what I did… I am not sure if it will help but this is what I did…

What I filled out and did to get disability…

COPY EVERYTHING YOU SEND THEM AND THEY SEND YOU…smiles.

* Filled out and copied the disability report for the ssa… and on every single question I read I could explain on I did.

* OHSU Fibromyalgia Clinic Questionnaire – this is a 10 page history of your life and pain… when I finished with it I had 20 pages…. tell them everything you can think of and explain as much as you can. (filled out 2 times…once when I first sent everything in and then about a month later.)

* FIQ – Fibromyalgia Impact Questionnaire -- filled this out once a month

*Social Security Statement – this they send you atleast once a year…send them all o them you have… keep copy for yourself.

* if you get help from the state you live in copy and send all of that paper work too… attach a letter tell how, when, and why you have this.

*Bank statement and voided check

* birth certificate and social security card (copy)

*keep copies of everything they have you sign

*resume and list of every job you have ever had (reason for leaving job, time on job, pay)

*articles on what is wrong with you… anything and everything you can get your hands… helps them to understand where you are coming from (keep copies for youself)

IMPORTANT: Make sure you are keeping track of phone calls, times of appointment, date on everything even on when you send all the paperwork in…

* pain journal… I kept one for the whole process and would send the DDS (Disability Determination Services the pages every few weeks.

*okay this one is weird but it is what won my case…
Anxiety, Panic, Depression, and Rage -- now this is a parkinsons work sheet but it was used in my case to get me into a shrink to see if I was depressed or had anxiety

*Called all my doctors and asked them to send all the information that would help your case to your SSDI worker and you DDS worker… and what I did wrong on this part was that I should have gone to the doctors (all of them), get all the information (yes it will cost to get the copies), make copies for yourself and then hand deliver the reports from the doctors to the DDS worker or send then in a priority envelope with a signature required – so you know it got there.

*when ever they give you a new med or take on away make sure to let your DDS worker know… this helps your case too…smiles.

*make sure you tell them everything… good and bad…. It will help…smiles.


One last thing… if there has been articles about ground water making people sick, or pollution in your area … anything that harms your health… put it all together…articles – everything and copy and send it all… you never know what will help… wont hurt you… smiles.
