Subject: Knitted tissue box cover

I want to knit a tissue box cover - one for a small, square box. I figured out how to and what size to knit the sides, but I'm not sure how to make the top, so that I leave a hole for the tissue to pop out from. Anyone have a pattern? Thanks!
