The way this works is that the crown (the winding knob) screws onto a stem that stretches down into the watch and attaches to the winding mechanism. The sleeve is a little hollow cone-shaped device, with flanges down to the tapered end, that screws onto the inside of the watch case. The stem slides through the inside of the sleeve, and the flanges hold the stem in place so it doesn't fall out when pulled up and also allows it to turn freely to wind the watch or set the time. It's possible that something's bent or binding with the stem and sleeve assembly, which makes it hard to pull up or down. (It might also be misaligned when the watch movement was put into the case - which is an easy fix but also something any good watchmaker should be able to do properly.)
