Here's a few differences I've noticed after reading the books and watching the anime.

1. Yoko goes to the world of the 12 kingdoms by herself. (Asano and Sugimoto don't go).
The story remains mainly the same, but some details are changed to accomodate the 2 new characters, i.e. Kourin was ordered to kill Yoko in the novel instead of Sugimoto, and Asano and his gun don't appear.

2. The shusei(traveling entertainers) don't have a major role in the Tsuki no Kage books.

3. Yoko seems to wander around Kou by herself for at least a month before she meets Rakushun.

4. When the stories are told about Taiki, and En, they are not being told to Yoko. They are just told like regular stories.

5. The story about the Emperor of En in Japan is told in Higashi no Wadatsumi, parallel to the story about Atsuyu.

Well, that's all I can really think of right now. The anime is a bit different, but I don't think that makes it any worse. I am still a big fan of the anime, and hope that they continue making it.
