2004 Beautiful Divas Calendar Contest

These Calendars Make a Great Gift!!!

12 Winners will receive their photo on a month on the 2004 Miss Diva USA Wall Calendar!!! Winners will also receive a Gorgeous Crown, a Lovely Custom Banner with the month that they win (�Miss Diva January�), a banner pin and a $50 certificate to be used at the 2004 Miss Diva USA National Finals!!!

Entry Fee: $30 for the first photo
$10 for each additional photo

Photo can be color or black & white and should be no smaller than a 5x7 and no larger than an 8x10.

Please write the name, age and address on the back of each photo.

If you would like to have your photos returned, please include a self-addressed envelope with sufficient postage.

Please be sure to package photos carefully, we cannot be responsible for photos damaged during shipping!!

All entries must be received by November 15, 2003. Any photos received after November 15, 2003 will not be entered and will be returned to you.

Winners will be notified by November 30, 2003 and calendars will be mailed by December 15,2003 in time for Christmas.

I hereby agree to hold Miss Diva USA Pageants, director, staff & facility where pageant is held, completely harmless for any unfortunate events which occur before, during or after this pageant, including, but not limited to theft, illness, injury, accident, death, act of war, act of God, etc. I understand I enter this pageant at my own risk and assume full responsibility for my property and person. I understand that decision of the judges is final and unsportsmanlike conduct (before, during or after the pageant) may be grounds for disqualification without refund, and/or cancellation of an awarded title and royalty status. I understand that no refunds will be given unless the pageant director cancels this event. I give full permission for photos & videos taken during this event to be used in promotion of the pageant system, including names and likeness. I take full responsibility for any copyright issues on any photos I submit for the program book.
