I have not heard of this happening before, the just like humans, some dogs are very intolerant of medicines, etc., especially if there is an underlying illness or genetic problem that may not have yet been detected. Did you have a necropsy done on the pup to determine the exact cause of death? If not and it's not too late, I would suggest doing so.
