Well the main problem is that most major or large ISPs (and even now smaller ones as well) are using what are called caching proxy servers, they are invisible to the people using the ISP, but not to places running servers. AOL is a great example of this, they have 14 million+ users but all of them will show up from a few dozen servers, probably with webcache in the name somewhere. So if one user from this ISP is giving you problems you are not really blocking that user, but EVERYONE using that ISP because they all share maybe 1, 2 or dozens of servers. I have a 1mbps bi-directional connection with US West with static IP addresses. Incoming traffic TO my IP addresses works just like it should, and I run web, dns, email and ftp servers on these addresses, BUT when I visit web pages you will see me coming from only a few server IP addresses with webcache also in the name. ISPs do this of course to speed up your time on the net by caching unchanging web pages that a lot of people maybe visiting.

but the short of it is, if you have a person you are trying to ban from these servers, you will end up banning everyone that uses that ISP, there really is no way around that, because it does not matter if the ISP has 10 or 10 million users they will all be coming here looking like the same few IP addresses as the caching proxy servers.
