I basically agree. I hate it, however, when a player does something like this:

"I swing wildly at his head with my ax as he ducks out of the way but the return stroke rips a hole in his chain shirt and the sound of ribs cracking split the air -- I deal 8 points".

Who is he to dictate what just happened to an NPC? What if it was freaking Elah? Do you see what I mean? The sound of ribs cracking is somewhat presumptuous and should be left to the DM in order to maintain verisimilitude.
