Tonya can probably answer your questions more thoroughly than I can because she is a nanny to multiples but I'll give you some basic answers based on my knowledge and from talking with other nannies.
Yes, you definately need a contract. It protects you both from changing things in midstream without consent.
I have a friend who is a nanny in VA (similar area so I'm guessing similar pay scale). She is a live out and is paid $15 an hour before taxes for caring for a single infant (I would assume a slightly higher pay rate for twins but not double.) Plus the family provides a car for her to use when she needs to take the infant to "Mommy and Me" classes, etc.
As far as vacations go, you'll need to work that out with the nanny in your contract but I would say that she should get a week a year at minimum paid at her average weekly rate.
There are sites linked to this website for searching out a nanny without an agency, I have no experience with them though because I was placed through an agency.
