I know it does not do you much good now, but the guide is going to be quite necessarily updated starting this fall. Hopefully in print sometime in the spring. All of your questions are good ones and will be addressed on the new guide. Sorry I don't have answers now.

As far as starting upstream of Johnstown at Greenhouse Park, the Ferndale gauge should be over 3'.
