And I would imagine it is similar elsewhere. The law says that one parent may file for the name petition, but then later on it says that if the bio parents are not married or are not together, that both parents must submit the petition.

So yes, at least here both parents need to agree to a name change. I would imagine this is pretty similar elsewhere. The law here says for anyone under age 14, so I am assuming that at age 14 a child may make the petition for him/herself.

I also think at that point you don't need to legally change it, I think you can do a common law name change and have all your records changed... as long as it's not for fraud ;)
