if you follow the seasons arc the asylum was just a device to show Buffys depression and estrangement from her friends and was therefore not 'real'.
If you go with the lighting. The scenes in the institution were Harshly lit with a lot of blue and looked unnatural the 'Sunnydale scenes were lit with much more natural tones
and looked more real. showing joss's intent. However if you follow cold logic Buffys life in the asylum is reality and life as the Sunnydale slayer is her delusion
