The PTB Laws have been so relaxed that this is the result we get. The laxness in allowing the ball carrier to move forwards off the mark has encouraged this to be the norm. It was also partly why they don't play the ball with the foot: too busy moving forwards. The forwards movement then doesn't give the markers room to stand in the proper position and we see penalties for the markers not being in the correct position.

Of course, this could all be traced back to allowing coaches to influence the interpretation of the PTB as something to be won and rights given to the ball carrier.

Laws of the Game Section 11 Part 10
(d) The tackled player may not play the ball before the players effecting the tackle have had time to clear the ruck.

That is perhaps one of the most forgotten Laws in the game!
