It sounds like Harpo may have Swim Bladder Disease. It is especially common in goldfish. Dry food that is eaten too quickly swells up inside the fish's intestine... making them look pregnant... and keeping the fish from controlling their swim bladder properly... (so they float sideways and upside down)

This can be helped with antiobiotic food if the fish is still eating (feed Harpo pre-soaked or gel-based foods... peas are good too), otherwise use antibiotics should be added to the water of a quarantine tank if the fish is too sick to eat!
