Some comments

The boy certainly had other tests. His mother almost certainly knew, and probably had signed a consent form for the other tests.

Since we don't know if the boy had had other medical problems, we don't know the reasons for the tests. Did he have other medical problems? Was this part of a study? Why could a puberty assessment not have been done by his regular doctor? That seems to be fairly routine, and there would be no problem explaining to the boy why it was needed. While I certainly don't doubt that something happened here, there seems to be much we don't know or that the boy himself does not know or remember.

It seems there is a considerable potential for psychological trauma in this situation.

The new hygienist was almost certainly at least a nurse, and if she was performing the examination in 1970s, probably a doctor.

One possibility to consider is the boy was given ketamine as a sedative for the procedure. This drug produces unpleasant dreams or hallucinations in a small percentage of patients as its the effects are wearing off, and parts of this story could be dreams or exaggerations brought by ketamine or another drug. It may be noteworthy that the boy's account seems to indicate that the "hygienist" had him strip and began a physical examination after the anesthesia; this seems to me, at least to be out of the proper order in time, but I could be wrong. We have no evidence he received ketamine or another drug with similar effects, but IMO it should not be completely ruled out or discounted as a possibility.
