Many questions at once!
First of all, I have to say that diaper education is always very individual and you always have to weigh up which is useful/possible in the personal situation.
My children: I myself have an almost 11 yo. daughter, but some nieces (younger and older) that are raised similarly/as well.
Plugging: I set the plug for my girl from about 8. First at times, then more and more. I presented her the choice: To make #2 in the diaper or to always empty herself with the help of the plug at the same time.
At school she is mostly without plug but with a diaper (Pull-up with mild booster), but immediately when she is at home she gets the right house diaper and of course the plug.
When she gets older - so >14 - we will see how things go. Actually, she should stay in diapers until 18.
