Howdy Brian;

I can't find a photograph of SP5 Janis Miculs posted online, but his Army Serial Number is RA 16878675 (which means that he enlisted) and his MOS was 67N2F (UH-1 Helicopter Repairer). He was born on 4/29/45 and his hometown of record is Grosse Point Park, MI. His tour of duty in Vietnam commenced on 12/14/67 and he was assigned to the 155th Assault Helicopter Company, 52nd Aviation Battalion, 17th Aviation Group, 1st Aviation Brigade. He was KIA on 4/19/68 (not 4/19/69) in Phu Bon Province due to "Hostile, Helicopter-Crew, Air Loss, Crash on Land". The helicopter was an UH-1H (Huey) with a tail number of 67-17255. Note: Phu Bon Province is now called Gia Lai Province and is located in the Central Highlands of South Vietnam and Pleiku is its capitol. SP5 Miculs' religious preference was Roman Catholic, he was single, and his body was recovered.

There may be some other information available from the accident report for the helicopter crash especially in connection with other casualties. In order to obtain a photograph of SP5 Miculs, you will have to contact the guys of the 155th AHC or try his high-school yearbook from Grosse Pointe Park, MI. There was a tribute to SP5 Miculs posted by a fellow high-school classmate on the Vietnam Veterans' Memorial Wall. Perhaps he has a copy of the yearbook. The high-school library will undoubtedly have a copy as well.
