This year TXF submitted Theef, First Person Shooter and The Amazing Maleeni. Seems like odd choices but they did win three Emmys. FPS won two and Theef won one for makeup. Maybe the behind the scenes crew people get to submit their own epiosdes.

I think the actors get to submit episodes too. Seems to me I read that they were trying to drum up support for a nomination for William B. Davis for En Ami. And I think DD and GA get to chose an acting ep for themselves each year but I haven't heard what their choices were for season 7.

I don't know about Mitch but I think it depends on who decides to submit something for consideration, whether it's the show, the TV studio or Mitch himself, I'm not sure. But I don't think the Emmy panel just looks at every show and says, "Oh he was good." A person has to be submitted and get their name considered and try to get a nomination.
