Subject: More on Pegasys from Washington Post

The study shows that genotype non-1 patients will do as well as 78% SVR and
they can do this with 6 months of treatment and using 800mg of ribavirin
with Pegasys. They did this with lower adverse events and withdrawls than
genotype non-1 patients using higher doses of ribavirin or longer duration
of treatment and actually who got no added benefit of those higher doses or
longer duration. So these folks can use the lower doses and shorter
treatment duration.

Also very positive are the results for genotype 1 patients. For Genotype 1
patients, the best results for all came from staying on treatment for a year
( 48 weeks) and using higher doses of ribavirin (1000/1200mg/ day based on
body weight). Overall these patients got a 51% SVR; with Genotype 1 low
viral load patients getting 61% SVR and Genotype 1 High viral load Patients
getting a 46% SVR. These are the highest rates we have seen in this hard to
treat group. Genotype 1 high viral load patients are the majority of HCV
patients in the US.

Pegasys is anticipated to be approved by the FDA in the later part of this
year.

What is the reason re-treaters respond at the far lower percentage than naive treaters? Is it because the first attempt at tx with peg-intron somehow mutated the virus, making it harder to treat the second time around with pegasys? ...ooorr...is it because of a tougher strain to begin with that wouldn't have responded to pegasys either?? The reason I ask is important. If there is a good chance that a first stab at treatment with peg-intron would lessen my chances at a response to pegasys...I'd wait! (noooooo...I'm not trying to put it off again!...lol

That's the million dollar question! It's my belief, based on nothing anyone should trust, that it is because the virus mutates from a failed attempt at treatment. Now, that being said, I don't really believe that the few percentage points in difference between the two pegylated products is significant. You are waiting anyway so my suggestion would be when it gets close to the time you are going to try treatment, that you ask your doc what he/she has heard. And who knows, Pegasys might be approved by fall.

I love that - "based on nothing anyone should trust"...lol
Wellllll....um, how about.....your opinion maybe? ...I know I trust that!!...lol...that just hit a funny bone...lol
Anyway...thanks for your opinion, I tend to feel that way as well, based on nothing at all...lol
I'll wait until tx time and see what's up then..thanks!

Hi Susie,

I tend to aggree on the difference on the few % points. However, I wonder about the side effects. I was under the impression (I thought, but may be wrong) that the sides were less on PEGASYS than PEG-INTRON. Anyone got any information on this or is it a grey area which is extremely personalized ?

I'm just reporting what we were told. The dropout rate due to sides and serious adverse events was 14% on the 48 wk arms and that is approximately the same as the PegIntron studies show. I don't know anyone who has done both Pegs so i have no idea.
