Subject: Assessments

I am having a problem doing my assessments and how to write them....Our instructor isn't the greatest help with any of this....she just throws us out their and then ask us to tell her what we did wrong...If I knew what I did wrong, I wouldn't have done it in the first place.....Help......how do you perform you head to toe assessment and how do you write it??????? When I do mine I feel as though I am probing the patient...any suggestions....how to make this more comfortable for me?
