Growing up in the 60's and 70's, this game, though considered a "natural" part of the innocence of childhood, was forbidden. This fact never detoured us from doing it. Knowing it was "naughty", we had little trouble finding secluded places to play.
For us, there was little interest in fronal portions of each other's anatomy. Our "exams" focused mainly on the bottom and upper thighs and included rubbing, poking and tapping. Since in role-play we were just emulating what we would experience at the doctor's office or hospitals, we would include these practices in our play.
Back then, Penicillin was the only medicine available as an antibiotic, and it was always administered in the bottom, that's how we did it in our play.
Also, rectal temperatures were commonplace for child exams back then so that became a part of our play exams as well.
We always played in seclusion since getting caught doing it usually carried dire consequences. Some kids played it with the patient nude but for us, we knew that nakedness was Harder to cover up if caught than just having pants down. Besides, the removal of clothing from the area to he examined was part of the fun and excitement of the game.
