I support a General Membership Meeting of the P.R.C.A. according to Rule B1.9.1. If I am unable to attend said meeting, I authorize my vote by Proxy.

KNOW ALL PERSONS BY THESE PRESENTS, that the undersigned members of the P.R.C.A., Mike Johnson, Jim Warren, or Fred Whitfield shall act and vote as my proxy on my behalf at the general membership meeting of the P.R.C.A.
