Depends ... on what has happened by the time the enemy
AFV decides to fire. Note that a crew must be present in
the hex to hitch/unhitch the ATG, rule 48.63.
Some details.
1) If the enemy AFV fires as soon as the HT expends its
first MP in the hex, both crew and ATG will be destroyed
along with the HT.
2) If the enemy AFV fires after the crew has unloaded (2 MP
for the HT), then the crew will have become a separate
target and would not be eliminated with the HT (although
they would need to pass an NMC per rule 32.5).
3) If the enemy AFV fires before the full (half of MP) cost
of unloading has been expended, then the ATG is destroyed
along with the HT. After that point both the crew and ATG
would have been unloaded.

Yes, as long as the leader did not direct any other fire
during the Prep Fire or Advancing Fire phases.

Not during that Advancing Fire Phase. Squad has to portage
the SW to remove them from the HT, so this would violate
rule 5.74.
