Modern Trends in Well-child and Well-teen Assessments - as They Manifest in Certain Areas in th World

A yearly well-child health assessment is standard in many societies. This assessment includes typically an assessment of general bodily

development. In addition there is often a genital and a rectal check that may be more or less thorrough depending on the age of the child and

depending on the standard in the particular society. In many societies this check is totally omitted, or it constitutes only of outer palpation with

fingers, retraction of foreskin for boys, spreading of labia for girls and spreading or the buttocks for both sexes.

There is however a growing tendency in many societies to call in children at specific age levels for a more thorrough development and intimate

exam in addition to the ordinary well-child physical. This thorrough assessment is typically performed in a health center or in a hospital, and

takes typically 2-4 hours.

The elected age levels for this type of exam tend to be 6-7, 11-12 and 15-16. The following is a survey of the types of specific exams that

probably will be included in this special exam session within few years in many societies around the world.

In some societies the procedures decribed are allready semi-standard, performed at so wide indications that around 30% of all kids go through

it, most typically at the age 11-12, and they seem to be repidly developing into standard exam protocol for all kids.

The following information is based on extensive polls made about the exam types people in various countries experience at specific age levels,

and on facts regarding the technological development in examination equipment. The details specified are only meant as a documentation of an

ongoing development. They are not meant as some kind of advice about what type of exams children should go through, or as any judgement

weather this development is good or bad.

At no place are absolutely all of the procedures listed in the following done. Any place that perform such extensive ordeals on children will

choose some of these procedures, but which of them that each site selects, seems to be somewhat arbitrary.


EXAM INITIATION

In some societies the parents will be instructed not to give the child food after some time the day before and to give the child some laxative

agent to make his stomach empty.

This special exam is likely to start with an structured interview with both the parent and the child, and with cognitive tests. During this part the

child will be fully clothed, but in a light way, so that the staff can assess bodily reactions to posed questions. At the end of this preliminary part

the child will be clothed down to underwear.

The exam is likely to proceeed with anthropomorphic measurements with an assessment of the growth patern. This stage will include

measurement of weight, height, leg length, and circumferance of all body parts. There will also be measurement of subdermal fat thickness.

After this part the chilld is likely clothed down to only briefs around the genitals.

Then there will be assessment of body functions like gait, active flection, ballance, coordination of limb movements and muscular strength. Now

there will also be a neurological assessment where the sensitivity in all body parts will be assessed by gentle stimulations. The neurological

assesment will end by the lower underwear taken off so that the child is totally naked and the sensitivity of the outer pelvic areas tested. The

child will remain without ordinary clothes for the next stages of the examination, but will be offered a gown or carpet to warm and shield when

total nakedness is not necessary.

ORTHOPEDIC EXAM

This stage to come now will be a thorrough orthopedic examination, where the shape, flexibility, function and development status of all muscles,

joints and bones in the body will be assessed, including the whole pelvic area and the muscles in the pelvic floor. This examination wil include:

-A check of spine curvature to diagnose scoliosis or other deformities, performed with the child in standing and foreward bent posisions.

-Checking of flexibility in the schoulders, armes, spine and hip with the child in standing and sitting posision.

-Assessment of leg and hip joint flexibility with the child laying, performed by probing all possible modes of bending and twisting of the legs.

- A general inspection of the pelvic structure with the legs bent up and spread, including assessment of the alignment of the rectal opening,

symmetry of the buttocks, and symmetry of the outer genital structures.

-Ultrasound examination of the major joints in the body, especially the hip joints, knees and shoulder joints.

-Additional ultrasound check of body areas where the forgoing examinations have revealed symptoms of possible problems.


ANESTHESIA

In many societies the child will be given some degree of anesthesia for the rest of the exam, most likely only some calming medication through

the mouth or light sedation through an intervenous catheter, or with gas through a mask. The intention of the anesthesia is to make the child

calm and relaxed, and to hinder the child from remembering much of the more intimate parts that follow.


ASDOMINAL EXAM

After the orthopedic examination there one will perform an outer abdominal assessment whith the following procedure

. Manual palpation and percussion all over the abdominal and renal region with the child laying on the back and laying at the sides with the legs

flexed, to assess the shape of the abdominal organs, detect any tissue changes, painful points, muscular tension, masses, fluid accumulation

and abnormal accumulation of intestinal content.

- Ultrasound examination of the abdomen with special attention at the bladder and the renal area.


GENITAL AND URINARY EXAM

Then there will be a more specific genital assessment with the child laying on the back with the legs flexed up and separeted. The exact posision

will vary according to the development stage of the child. For girls this will include:

- Manual palpation of the outer genital area

- Separation of the labia to inspect the inner area of the genitals.

-.Taking specimens form the outer urethra and vagina to analyse secretions and microbeal content.

- Inspection of the vagina with an optical scope.

- For girls in puberty vaginal ultrasound exam.

For boys the genital exam will include

- Manual palpation of the penis, scrotum, testicles and the area down to the anus.

- Retraction of foreskin and spreading uretral opening for inspection.

- Taking specimens form the outer urethra to analyse secretions and microbeal content.

- Taking specimen of prostate fluid with a tube inserted into the urethra, and if necessary with a massage of the prostate with a finger inserted

through the anus.

- Palpation of the prostate with a finger inserted through the anus.

- Ultrasound exam of the scrotum, testicles and the area down to the anus.


The child will then undergo cystoscopy where one inspects visually the inside of the urethra and the bladder, and possibly also takes small

specimens from the inside walls. For boys there will be a thorrough inspection of the prostate area during this examination. Traditionally

cystoscopy has been a difficult examination to undertake, but modern flexible instruments ha made it to an easy and very quick examination.

Therefore this type of exam that sounds very intrusive is very likely to be included. During this exam the bladder will be filled with fluid and in

that shape also assessed with an outer ultrasound sond.


EXAM OF THE RECTUM AND COLON

The physical assessment concludes with the rectal inspection, which will include

- Examination of the anus and interior room with an inserted finger

- Opening of the anus with a speculum for visual inspection and taking specimens.

- Taking specimen from the rectum with an inserted instrument while the speculum is in place

- Flushing the rectum and lower colon clean out content with by means of an inserted tube.

- Internal ultrasound exam to visualize the organs around in the posterior pelvic zone.

- Inspection of the deeper parts of rectum and terminal colon with a flexible optical scope

The instruments used in most of these examinations will take pictures and measurements that are transfered to a computer, stored there and

later taken forth for further inspection and diagnosis.

The activity I am talking about is trade of organs and tissues - mostly from children.

And organ trade by private gangs is only the tip of an iceberg. There is an even more sinister organ trade going on by governmental hospitals

and other official health institutions.

There is an International official network of organ exchange with official hospitals in each country as nodes in the netwerk that provide and

resceive organs and tissue for transplantation, treatment and research, and with officially appointed coordinators at each site.

These coordinators work directly together with doctors responsible for treatment in the same hospitals or cooperating hospitals.

The function of this network is officially regulated by laws of each country. These laws genereally state that one shall only exchange organs from

people having been decleared brain death.

But the demand for organs and tissue, and especially from children of young persons as donors, are many times higher than that which can be

provided by persons being brain-dead after accidenst.

Many symptoms point to the fact that this network since long has gone corrupted due to the high demand and to the money exchanged for

organs provided.

The corruption occurs by taking out undamaged organs from patients that have been injured in accidenst and selling these through the

international network. When doing so they claim to the patient himself, his parents, his wife or relatives that the organ was damaged and had to

be removed.

The corruption also occurs by declearing brain dead people that are not so, and using their organs for trade in the network. The corruption

manifests itself with several symptoms any citizen can observe:

- Organs are taken out of a person after an accident that did not seem to injure exactly the place where the organ is situated.

- A stragely great persentage of accidents in a country or area result in removal of organs.

- A strangely great percentage of accidents of all kind end in death, despite the modern technical abilities to save persons after an accident.

- People in a given country are astounishingly often pronounced dead, even after accidents that do not involve greater part of the body.

- One have a routine of giving up the fight for the life of a person, and declares the person brain death, after very short time, usually 4-8

hours.

- Young persons that are supposed to have good chances of surviving an accident is surprizingly often decleared dead.

- Persons are often decleared brain death after accidents where the body, and even the head is largerly undamaged, when the person is without

consciousness after the accident.

- Persons are routinously subjected to artificial coma after accidents, even when the person was conscious right after the accident, and after

some hours decleared brain death.

- Parents, sposes and relatives are rutinously denied access to a person after accidents.

- Politicians in the country are actively demanding more organs from own citizens to be provided for donation.

I have for a long time observed these particularities in my own country, Norway, and I am concerned. Norway is a member of the international

organ donation network. But since this network is international, the concern is valid for several coutries, surely.

Norway is generally regarded as a wealthy country with few economical problems that will stay clear of the ongoing decline in global economy,

but it is not so unfortunately.

Due to a huge income from oil and gas fields which gives the Norwegian goverment the ability to collect great tax amounts and gather huge

funds for future prosperity, the world is generally regarding Norway as a country without economical problems or difficult challenges.

Unfortunately, the situation is very different. Here is a survey of the most important aspects of the economical situation of Norway as by spring

2014:

The good side of the situation are still huge annually income from the oil and gas fields and one has found even new fields that can be opened

for exploitation. Another good side of the situation are big funds that still are growing by saving 96% of the annual tax income from the oil and

gas production. It has been an aspiration of the politicians to make the funds so great that interests and dividends from the funds can pay for all

pensions payed out to Norwegian citizens anually This aspiration is still far from fulfilled though, and gets steadily more difficult to satisfy due to

a population growth generated by foreign immigration to Norway.

is not very rich.

There are emerging serious issues regarding the sources of economical income that Norway has available:

- A great part of the industry is directly engaged in the search for oil and gas, or in the production of oil and gas. Another great part is engaged

in support of the oil industry, either directly or indirectly. Even airways, shipping, hotels and restaurants belong largely to this part.

- The oil an gas prizes are falling, partly due to the economical world climate, but even more to huge new fields that are been opened in other

parts of the world, and even greater fields that has been found which still wait to be opened, for example in Australia.

- The Norwegian oil an gas fields are running out, which has the effect that the oil production has decreased by 30% from the top by now, and

the new fields found are not great enough to compensate for the loss.

- The cost of readying new fields for production and the production itself on Norwegian sea territories are skyrocketing, because the oil is

steadily more difficult to bring up. The oil istallations also by now have a dangerous maintainance backlog that often creates production stops.

- These three facts above make the proffit from Norwgian oil and gas production decrease year by year and there is a real danger that the

whole of the Norwegian hydrocarbone production will be proffitless within few years. This will have the effect that the whole of the industy that

supports the hydrocarbone production will lose its proffit too..

- Norway has some industry in addition to the hydrocarbone dependent sector, mainly production of raw materials like aluminium, titanium,

fertilizers and electric energy. There is also a smaller sector of specialized mechanical industry, information technology, furniture and

biotechnology. But these industries are heavily afflicted by the general worldwide recession that does not seems to end for some years. The

proffit ifrom these industries are generally declining and is suspeced to decline for years to come since these industries are equally affected by

law economical activity like the industries in the rest of Europe.

- Norway also has let the competancy in other industrial fields than the hydrocarbone-related ones degrade and there is little innovation in

other fields. Norwegian industry is also getting constantly depleted by owners selling the share majority to foreign interests that after some time

take down the factories in Norway and bring the patent and compentency out of Norway.

Norwey also have some huge internal challenges, directly connected to the type of society that Norway has become.

- The country has perhaps the greatest and most expensive burocracy in the whole western world. A huge part of the Norwegian population is

engaged in controlling the rest of the population in various ways. The average Norwegian citizen does not, however, feel vey much in their daily

life from the presens of this burocracy. It mainly stays in the background, waching everything and everyone and collecting data about everything

and everyone. Nearly everything a Norwegian citizen does is in some way recorded by the burocracy..

- Norway also has a huge social and health sector, which however is very inefficient and generally are not solving the task they are meant to

solve, and to some extent create broblems rather that solving problems. A great part of the Norwegian population is at any time out of work,

waiting to be treated, or they are under treatment that does not cure their health problems at all, or only very slowly, and often causes new

health problems.

- The country also have a great privat sector that delivers social services, including child protective servcises, and health services ordered by the

public social and health sectors which burden the economy..

- The country's infrastructure is mostly worn down. There is a huge backlog in the maintainance of public buildings, hospitals, schools, railroads,

sewers,electric grids and roads, and the backlog is growing.

- The Norwegian home prizes are among the highest in the western world, to some extent due to a backlog in house building in press areas, but

even more due to a joint colaboration between real estate brokers, bankers and speculants in bringing the prizes to the greatest level possible.

- The average Norwegian citizen has a dept, mostly due to high prizes of homes and cars, that they are not capable of serving if the interest

rates get higher or the average income decline, both of which is highly probable. A great part of the Norwegian population is also in a situation

where they pay off dept by establishing new and higher dept. The same is true about Norwegian industries.

- There is a constant immigration of people from foreign culture.s When coming to Norway they will have a long time to adept and the

Norwegian society has huge costs for the integration of immigrants. Many of these come from conflict areas and come here with great

psychological trauma. Not a few also come with the attitude that Norwegians are obliged to adept to the rather strict living principles of their

religion. These issues and the conflicts they create has become a constant challenge in everyday life of Norway.

- But Norwegian employrs also have got the habit of importing workers, and squeezing out people alreathy established in Norway from the

workplace, in order to get a cheap and easily manipulated workstock, and thus contributing to a growing rate of unemployment.

- Norway has a very high de facto unemployment, but most of it is redefined to other cathegories and the unemployed giving permanent

welfare. The official unemployment rates are rather small, but does not give a correct picture of the situation.

The economical funds possessed by Norway seem big, but they are to a great extent invested in precarious areas, including loans to countries in

utmost difficulties. The funds will experience great losses, but how great is difficult to say, but the true value of the fuinds are surely much lower

than the figures show. The funds are however not as huge as someone believes. If the whole of Norway was to live off the funds, they would

only last 2-3 years, actually. Furthermore, Norway also have dept towards foreigh interests, mostly privare dept, that is nearly as big as the

funds, which means Norway is only marginally a net creditor, or more probably a debitor if losses from the funds are taken into account.

It is higly probable due to the listed factors that the average income per capita in Norway will decline drammatically in the near future. Then a

huge part of the population will not be able to serve their dept and the government will not be able to maintain the huge public sector,

comprizing also the private sector that delivers services to the public sector. The government will neither be able to alleviate the situation very

much by using the funds either, simply bcause the funds are far from limitless and very illiquid.

One can hardly draw any other conclusion out of these facts that the Norwegian society will experience a collapse of the kind we have seen in

Hellas or Spain, whith all problems herein included.

Norway is seen by many as a paradize, but the country has not actually done it very well despite the huge income from oil anfd gas fields. Right

now the situation is like this:

The Norwegian pirvate sector have foreign depts that are nearly as big as the governmental funds. In addition much of the funds are invested in

precarious areas, so a lot of it is to be regarded as lost or at best highly illiquid. When everything is summed together, Norway is far from any

rich country.

By now the proffit from the oil and gas is rapidly declining, due to decreasing oil and gas resources, to falling oil and gas prizes and to

skyrocketing project and production costs in Norwegian fields.

The rest of Norway's industries is not prepared for the current development on the Word markets. Nearly all industrial sectors are oriented

towards oil or gas in some way or another, or towards building projects, which is not what the world is doing right now.

In addition Norway has a very heavy and costly public service, and the average Norwegian family is heavily in dept due to high costs of

houshold. A surprizing fact for many will be that also the infrastructure of Norway is heavily worn down.

By studying the philosophy of football managers, you can often deduce a lot about the society where the managers have been rised.

Managers having a rigid philosophy that allow very little individual choise and artistry for the players, are often rised in a very authoritorian

Much of the same can be said about English and American football managers, but those show openly their authoritarian attitude, while

Norwegian managers hide it from the surface.

Managers from latino countries may appear authoritarian at the surface but they allow the players to develop their personal artistic style and

allow thw team to take decisions as the play goes along.

It is obvious, based on a long history, what kind of managers and what kind of players that make the best results, namely the latino type.
