If the skin is bleeding, there is more going on than just acne/blackhead issues. I would take the dog back to the vet and get a skin scraping done. At minimum, my guess is this poor kid has a staph infection going on and needs to be on a minimum of a 21-day antibiotic regimen.

You also should take a look at what you're feeding as some (or all) could be directly linked back to diet. Try to feed a premium food with little to no grains and I would even opt for a novel protein source (potato/duck, fish/sweet potato, venison or bison are a few that come to mind). Many of the prescription shampoos can help, but they can also exacerbate the condition because they are so drying. Be sure the lotion your vet gave you does NOT contain lanolin.

I would start with a skin scraping and go from there.
