I think in this situation I'd want to either talk to the owner's vet or get my own vet to contact the owner's vet. I'd really want to know why one of my puppies died so soon after I homed it, if indeed it did die just 5 days later. To be honest I'd want to know why any of my dogs died at whatever age, but I do understand that if the owner isn't speaking to you, this could be difficult.
Perhaps offer a refund subject to cause of death being confirmed by way of vet letter or Death Certificate? Do you really know that the puppy did die? Has any proof of anything been given?
These matters are best resolved between the parties concerned and only tend to hit the "boards" when an amicable solution hasn't been found.
I don't really understand the replacement puppy offer, surely if you have worries about this owner and they have already had one of your puppies die and you now no longer have a good relationship, do you really want to send them another dog? Wouldn't a refund or partial refund give you better peace of mind?
