Did you install the roof up underneath the vent before you sat the flashing on the roof deck?

If its one of those huge flashings with only a 3" lip around them you would have wanted to run a bead of roof sealant around the edge of the top and sides of the roof flashing before setting the roof material on the lip of the flashings.

When you installed the material over the flashing did you make sure to NOT drive the fasteners you where using to install the material too close to the water line of the flashing?

It also helps to leave a 1/2" gap around the water line on the flashing rather then butt you material to the flashing.

If you are a novice its possible that it might not have occurred to you to maintain your two layers when you went over the top of the flashing and it could be leaking through the gap of the tabs on the top of the flashing.

These are some of the most common reasons I have run across for the failure I believe you are referring to.
