The idea that humans are living in some kind of dreamworld or some kind of glass tube experiment that we can't see, has been played with many times in sci-fi. The most recent and popular film versions of this can be seen in the Matrix and Dark City where the heroes become aware of the fake-ness of what they perceive to be reality.

While this is an intriguing though about what Duncan Idaho's graphical vision could mean, I doubt that this will be the case because evidence points to something else.
