Incident 1 - The penalty for "inside the 10" in the field of play would be where the player is standing inside the 10. You cannot go to a specific spot because "the 10" is a circumfrence around the player receiving. So if he is inside the 10 in the in-goal it would logically be 10 metres out in line with where he is.

Incident 2 - Same as any other incident of foul play in the in-goal or any other deliberate breach, 10 metres out in line with where the incident occurred.
