From my experience, I don't believe that thin cotton underwear makes any real difference in terms of pain perception. At least not when the spanking is administered with a wooden spoon, paddle, belt, or even a cane. I don't have any reliable experience in this area, as in my family we always got spanked / spank on the bare bottom, except for the occasional friendly pat.
First and foremost, there are probably two differences. From the child's point of view, spanking on the underwear at least preserves the visual protection of the rather intimate parts of the body such as the buttocks, genitals, or even the butt hole. This reduces the feeling of shame, total defenselessness, and utter helplessness that always overwhelms you when you are being spanked.
From the perspective of the person administering the punishment, a completely naked bottom makes it easier to assess the effect, as the condition of the skin and bumcheeks is more visible.
And, of course, there is also the combination where the punisher deliberately wants to induce this feeling of shame, helplessness and defenselessness as part of the punishment. Be that as it may, after the first few spanks, I didn't really care about the dress code of my butt anyway. I had other things to worry about.
