I think further to her reasoning, there is a paedagogic aspect that makes it preferrable to use the withdrawal of privileges over physical force. If the boy is made to take of his underwear and let the examiners perform the necesssary procedures on him, he can not externalise the responsibility for consenting into the exam, whereas if he is stripped and examined with force, he can blame others for the execution of the exam and close his eyes to the necessity of these medical procedures. The latter might prevent him for ultimately taking over responsibility for his hygiene and health, which should be the goal of the maternal interventions.
